SUMMARY:

Meloxicam is a nonsteroidal anti-inflammatory non-opioid analgesic medication used to treat osteoarthritis and rheumatoid arthritis. A treatment range is often between 5-15 mg PO qd with a max of 15 mg/day. Meloxicam works through reversible COX1/COX2 enzyme inhibition decreasing thromboxane and prostaglandins leading to antipyretic, anti-inflammatory, and analgesic effects. It is metabolized via the CYP2C9 and CYP3A4 pathways. The time to peak for tablets is around 4-5 hours with a half-life elimination of 13-22 hours. It should not be used during pregnancy and close to conception. Black Box Warning for cardiovascular thrombotic events and GI bleed/ulcerations/perforation. To avoid and minimize GI related issues take with food.

ANSWERS:  

  1. E) is correct. Although C and D are within the appropriate dosing range, this patient is pregnant and meloxicam should not be used during pregnancy and close to conception.
  2. False. Meloxicam reversibly inhibits both the COX1 and COX2 enzymes. 
  3. A and D are correct.
